Insurance & Payment Options
Figuring out insurance should not feel like a second full-time job. We’re here to help you understand your options before starting therapy—without promising coverage that your plan may not provide.
Insurance participation varies by clinician, state, specific plan, and appointment type. Let us help you check what’s available for you.

Insurance plans we work with
Mosaic Clinic’s existing payment information lists the following insurance networks or arrangements. These are examples to ask us about, not a guarantee that every plan or clinician is in network. Plan participation, including Medicare and Medicaid-related arrangements, must be confirmed for your specific situation.
Blue Cross Blue Shield
BCBS of Illinois, BCBS Community, and BCBS HMO (Advocate PCP) are listed among Mosaic’s arrangements. Network and referral rules depend on the specific plan.
Aetna
Published arrangements include Aetna EAP, PPO, POS, and Aetna Better Health. Confirm eligibility and clinician participation for your specific product.
UnitedHealthcare / Optum
Published arrangements include Optum/United, Optum EAP, and Optum through GEHA. Eligibility and mental health benefits vary.
Cigna
Published arrangements include Cigna POS, PPO, and EAP. Confirm whether your specific behavioral health benefits are in network.
Other listed arrangements
Molina, TRICARE, and the VA Community Care Network also appear on Mosaic’s payment information. Referral or authorization requirements may apply.
Medicare and related coverage
Ask our team about Medicare acceptance and whether your particular Medicare plan, clinician, service, and location are eligible. Original Medicare and Medicare Advantage networks may differ.
Important: An insurer’s name on this page does not establish eligibility, an active contract, approval for a visit, or a specific out-of-pocket price. Please contact Mosaic Clinic and your insurer before relying on any listed participation.
How billing works
In-network care
When your provider is in network for your specific plan and your service is covered, we can help you understand the expected copay, deductible, or coinsurance. Your insurer’s determination and plan terms govern your benefits.
Headway billing
Our existing payment information says Mosaic uses Headway for billing in Florida, Maryland, and Texas. Enrollment and clinician participation are verified individually; not every appointment is necessarily billed through Headway.
Out-of-network benefits
If Mosaic or your clinician is outside your plan’s network, you may be able to use out-of-network benefits, subject to your plan. Our existing payment page mentions Mentaya as one possible resource; eligibility and any fees should be confirmed directly.
Self-pay and sliding-scale options
Prefer to pay directly, or don’t have eligible insurance coverage? Mosaic Clinic offers self-pay rates starting at $110 for an initial intake appointment and starting at $90 for subsequent therapy appointments. We also offer a sliding-scale fee option based on individual circumstances and availability. Please contact us to discuss eligibility, your clinician’s current rate, and whether you’re entitled to a Good Faith Estimate under applicable rules.
What to check before your first appointment
We recommend asking your insurer—or requesting assistance from Mosaic—with these questions:
- Is the specific clinician in network with my exact plan and member ID?
- Are telehealth outpatient mental health visits covered for my location?
- What will I owe for a visit: copay, deductible, coinsurance, or another amount?
- Do I need a referral, prior authorization, or EAP authorization?
- Are there limits on visits or services, or restrictions for group, couples, or family therapy?
- If I have an out-of-network plan, what reimbursement rules or documentation apply?
You can share the name of your insurer, plan type, and state with our team through an appropriate intake process. Please do not put your insurance ID, date of birth, or detailed health information into ordinary email or unsecured contact messages.
Coverage for different therapy services
Coverage can vary even within the same insurer. Individual therapy, family or couples sessions, group psychotherapy, assessments, and other services may have different eligibility and medical-necessity rules. We can discuss the expected billing approach before you schedule.
Mosaic offers telehealth across states where our clinicians are licensed. Coverage depends on your physical location at the time of the visit, the clinician’s licensure and network participation, and your insurance benefits.
Common insurance questions
Does Mosaic accept my insurance?
Possibly. The insurer names above reflect Mosaic’s published payment information, but the most reliable answer depends on your exact plan, state, and therapist. Contact us to check before scheduling.
Do you accept Medicare?
Mosaic can discuss Medicare-related options, but participation needs to be verified for the specific clinician, location, and plan. Medicare Advantage plans may have different networks and rules from Original Medicare.
Will you know my exact copay?
We may be able to provide an estimate after eligibility checks, but deductibles, claims processing, plan rules, and coverage changes can affect the final amount. Your insurer makes the final benefits determination.
What if my clinician isn’t in network?
Self-pay rates start at $110 for an intake and $90 for subsequent therapy appointments. Sliding-scale fees may also be available based on individual circumstances and availability. Ask us about the rate for your clinician and possible out-of-network reimbursement. Our existing payment page also describes Mentaya as an option for exploring certain out-of-network benefits.
Can I use an employee assistance program (EAP)?
Some EAP arrangements are listed on Mosaic’s existing payment page. EAP visits often have their own authorization, visit limits, and participating clinicians, so please confirm details in advance.
Does insurance pay for couples therapy or groups?
It depends on the plan, the service’s clinical purpose, and medical-necessity requirements. Group therapy, couples counseling, and facility-based programs may be handled differently from individual therapy.
