Not a diagnosis: This questionnaire identifies symptoms that may warrant evaluation. ADHD diagnosis requires a fuller assessment, including history, impairment, and possible alternative explanations.

Check the box that best describes how you have felt and conducted yourself over the past 6 months.

0 of 6 completed

1. How often do you have trouble wrapping up the final details of a project, once the challenging parts have been done?
2. How often do you have difficulty getting things in order when you have to do a task that requires organization?
3. How often do you have problems remembering appointments or obligations?
4. When you have a task that requires a lot of thought, how often do you avoid or delay getting started?
5. How often do you fidget or squirm with your hands or feet when you have to sit down for a long time?
6. How often do you feel overly active and compelled to do things, like you were driven by a motor?

Your screening result

How it is scored: On questions 1–3, “Sometimes,” “Often,” or “Very Often” counts as a shaded response. On questions 4–6, “Often” or “Very Often” counts. Four or more shaded responses suggest symptoms may be consistent with adult ADHD and a clinical evaluation could be useful.

This result alone cannot diagnose or rule out ADHD. Consider discussing your symptoms with a qualified healthcare professional.

Understanding your next steps

ADHD can overlap with or be affected by stress, sleep difficulties, anxiety, depression, trauma, and other concerns. A clinician can help clarify what is contributing to challenges with focus, organization, and daily functioning.
